SAFETY INFRACTION NOTICES
Don't throw your hard-earned due diligence defence away because you did not discipline or neglected to document it because the incident was forgotten about by the time you returned to your office. There is ample room on this notice to note: Worker Name, Department or Sub-contractor, the Date, any ID Number, a Detailed Account of the Incident, the Worker's Remarks, Worker Signature, Supervisor's Signature, and Signature of Issuer.
At 3.5" x 5", the notice fits into a notebook or folds in half
to fit easily into a wallet. Each carbonless notice comes in
triplicate: one for file, one for the worker and one for the
supervisor or sub-contractor.
